Required Member Information.

COVID REGULATIONS (6/11/22)

AFS, Inc. employees in all locations (office as well as home and community) will no longer be required to mask, perform contact tracing, nor perform temperature checks (for both members and employees).

As with all infectious illnesses, all employees are still required to:

  • immediately report signs and symptoms of illness for both the employee and member immediately report any fever of 100.3 degrees F or higher for both the employee and the member immediately report confirmed cases of infectious illness/disease/condition for both the employee and the member (COVID, variants, flu, etc.) practice universal precautions at all times

  • use additional PPE (including masking) for both employee and member when there are signs, symptoms, or confirmation of any infectious illness/disease/condition.

AFS, Inc. offices are still closed to the public. Members, guardians, and/or employees should schedule planned appointments prior to visiting the office.

AFS, Inc. is committed to following the recommendations of the American Centers for Disease Control and Prevention (CDC) in response to COVID-19, the Flu, and/or any other communicable illness, disease, epidemic, and/or pandemic. For more information, please go to the CDC website.

REFERRALS, ENTRANCE, AND EXIT CRITERIA

Referral/Admission to AFS, Inc.:

Potential members must be approved for CES Waiver Services in the state of Arkansas. Upon approval for CES Waiver Services, any person and/or entity may refer a CES Waiver Services recipient to AFS, Inc. Once AFS, Inc. receives a referral, an inter-disciplinary team within the organization will review the referral to determine whether AFS, Inc. can meet the needs of the recipient. Once a decision is made, the referral source will be informed of the decision (the recipient and/or their legal representative will also be informed of the decision if possible).

  • AFS, Inc. has implemented a Waiting List for potential members. CES Waiver Services recipients are added to the Wait List based on the date/time that the referral was received by AFS, Inc. When membership at AFS, Inc. becomes available, potential members will be contacted on a first-come-first-served basis as determined by the date/time of initial referral.

  • Exit Criteria: AFS, Inc. does not determine who qualifies (or continues to qualify) as a CES Waiver Services recipient in the state of Arkansas. Members may exit from AFS, Inc. services for a number of reasons, which may include, but are not limited to, the following:

    • The state of Arkansas determines that the member no longer qualifies as a recipient for CES Waiver Services

    • The member chooses to transition to a different setting and/or a different service provider

    • The member no longer wishes to receive CES Waiver Services

    • The member moves outside the state of Arkansas

    • AFS, Inc. can no longer ensure the health and safety of the member
